We see all patients on an appointment basis so that we can provide the proper amount of time for each person. We endeavor to see all patients on time and ask that you extended the same courtesy to us. At your first visit, we would appreciate you arriving fifteen minutes early to fill out the necessary forms(medical history and insurance information).
Your first appointment may include an exam of your mouth for decay, and an oral cancer screening,x-rays are used to detect hidden problems.
If you need to cancel or reschedule your appointment, please do so as far in advance as possible. The office reserves the right to charge patients for missed appointments without 24-hour notice.
We make every effort to keep down the cost of your dental care. You can help by paying for your treatment at the time of your visit. If your treatment program requires several visits, you will be given an estimate and asked to make financial arrangements with a member of our business office staff.
Dental services are to be paid for at each appointment by cash, personal check, MasterCard or Visa. All emergency patients must pay for their services as they are performed on the first visit.
A $5 monthly rebilling service charge will be added to all delinquent accounts.
Dental Insurance is designed to assist you in paying part of the cost of dental treatment. Insurance is NOT designed to pay ALL of the cost of treatment. Most plans include coinsurance, a deductible, and other expenses which must be paid by the patient at the time services are rendered.
Our office will do our best to help you maximize your insurance benefits. Please remember that payment of your claim is a contract between you and your insurance company. Filing insurance claims is a courtesy we extend to our patients, but all charges are ultimately the patients responsibility from the date services are rendered.
Preventive care is a way of protecting the investment you have made in your mouth. We believe strongly in the benefits of regular preventive care and therefore, encourage all of our patients to return for periodic visits. You should make your next preventive care appointment at the end of your current one to maintain optimum oral health. Otherwise, you will recieve a written reminder when it is time to return. When your receive this, call us promptly for an appointment.
Protecting Your Health
For your protection, we use the most up-to-date methods of sterilization and infection control. We follow the infection control guidelines of the American Dental Association and the Occupation Safety and Health Administration.
We Recognize that emergency situations do arise and we will do everything in our power to respond to your problem as quickly as possible. We reserve special times for those patients with emergency needs, and you will be seen as quickly as possible.
If an emergency arises when the office is closed, please call 456-0436. The answering service will promptly forward your message to the dentist on call.
